Through the Jane Portal, you can:
Schedule a new DOT or non-DOT physical, drug test, or occupational health appointment
Reschedule or cancel an existing appointment
Update your contact information

Accessing Exam Forms & Certificates
After your appointment has been scheduled in Jane, our team will create your account.

Once your account is active, you can:
Complete your required medical history form (DOT: MCSA-5875)
Review or download your completed Medical Certificate (DOT: MCSA-5876)
Access documentation for any required follow-ups, exemptions, or renewals
