How It Works

1. Complete the Company Enrollment Form

Tell us about your business, your designated employer representative, and list your covered employees who will participate in the consortium.

You’ll be able to add or update employees anytime during the year.

2. Submit Annual Consortium Fee

After completing the form, you’ll be directed to pay your annual consortium fee online securely.

3. Employee Drug Screening at RYSE

Each employee listed in your enrollment must complete a urine drug screen at RYSE Chiropractic & Wellness Center as part of the initial onboarding process.

You’ll receive a confirmation email with a scheduling link immediately after enrollment.
